COPD High Risk Program

The COPD high-risk program is designed to identify those individuals who do
not respond favorably to episodic treatment interventions, who are high
cost and at greatest risk for frequent hospitalizations.
The two primary objectives of this program are:
- to educate and encourage patients to self-manage their illness, and
- to partner with the health care team including the patient, physician, home care staff and payor to effectively manage the health care needs of the individual.
Who can benefit from the COPD High-Risk Program?
The high-risk population can be identified as people who are:
- Over the age of 70
- Polypharmacy users
- Repeatedly hospitalized
- Suffering from a chronic illness
- Incompletely evaluated by healthcare professionals
- Often arriving at inadequate solutions for their problems
- Subjected to extended lengths of stay in healthcare facilities

- A more resilient response to acute exacerbation of a chronic condition which will lead to shorter and less devastating period of illness
